Please allow the International Center for Veterinary Services (ICVS) to recommend to you the following if you will be leaving the care of your pet with an Ayi or Driver or other household employee:
1). Provide an original copy of written authorization that this Ayi/driver/employee/representative is the ¡°legally authorized representative¡± for the owner. Please include his/her name, phone number, address, ID number (shen fen zheng or passport number) and photocopy of the ID (if possible). Have Ayi/driver/representative SIGN THIS ORIGINAL COPY. It would be ideal if this authorization letter were notarized.
2). Include the owner¡¯s name, phone number, address, e-mail, and all overseas phone numbers in this authorization so the owner can be contacted if needed. OWNER MUST SIGN ON THIS ORIGINAL COPY.
3). If Ayi is primary care provider for your pet, please also have a trusted friend, preferably English and Chinese speaking, check-in on Ayi and your home at least every other day to ensure your pet is in good health and properly cared for.
4). Inform your animal hospital of your travel schedule so that the hospital will be prepared in the event your pet is brought in for treatment or hospitalization while you are away.
5). Keep your contact information updated and provide it to all parties that might be involved, including your animal hospital.
We hope that these tips will help keep you and your pets healthy and provide you with peace of mind when you are away. Owners are encouraged to contact ICVS to request Hospitalization and Medical Emergency Authorization forms to register pets before traveling.
