The International Center for Veterinary Services (ICVS) would like to wish you, your family and your pets a very happy and healthy holiday season!

To ensure that your pets remain safe and healthy while you are away over the holiday, ICVS encourages you to register your pets with us before you depart for your holiday. REGISTRATION IS FREE!

1). Client (Owner) Information and Registration Form

2). Patient (Pet) Information and Registration Form

3). Hospitalization and Medical Emergency Authorization Form

4). Surgery/Anesthesia Consent Form

These forms will provide ICVS with your contact information and that of your authorized representative to allow us to provide medical treatment for your pets should they require medical services while owners are away.

Please fill out, SIGN and return these forms to ICVS by fax, email (scan/email) or drop off at ICVS, Monday to Saturday. Please contact ICVS reception with any questions!
